This agreement is between: Rachel McGuire, EFT Practitioner and Client

Responsibilities.

1. Practitioner agrees to maintain the integrity and privacy of the sessions and all information gained within.

2. Client is responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results. As such, the Practitioner agrees that the Practitioner is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Practitioner.

3. Client understands that Emotional Freedom Technique is not to be used as a substitute for professional advice by legal, mental, medical or other qualified professionals and will seek independent professional guidance for such matters.

4. Client agrees to communicate honestly, be open to feedback and assistance and create the time and energy to participate fully in the program.

5. Client understands that Embrace This Place Practice as represented by Rachel McGuire is not a licensed counselor, psychologist, therapist, medical doctor, nutritionist or any other medical professional. I understand that she has no formal training in these fields and she does not portray herself as such.

Services.

The parties agree to engage in a pre-agreed Package details listed within program description.

Procedure.

The time of the sessions and/or location will be determined by Practitioner and Client based on a mutually agreed upon time.

Confidentiality.

This relationship, as well as all information (documented or verbal) that the Client shares with the Practitioner as part of this relationship is Private & Confidential. The Practitioner agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Practitioner will not disclose the Client’s name as a reference without the Client’s consent.

Cancellation and Lateness Policy.

Client agrees that it is the Client’s responsibility to notify the Practitioner at least 24 hours in advance of the scheduled call/meeting if the client cannot make the session. Client will be responsible for the payment for a missed session if the client does not turn up or reschedule. Practitioner will wait for 15 minutes, before the Client is noted as not turning up for the session. Client understands and accepts no refunds will be granted after services have been rendered. No refunds or rescheduling for no-shows. Rescheduling may be done with 24 hours’ prior notice, subject to available openings in the schedule. Partially used packages cannot be refunded, but remaining sessions may be used by approved family members. 6 month programs must be completed within 30 weeks of purchase. If a client wishes to obtain a refund before services have been rendered, it will be issued minus any transaction fees or costs.

Limited Liability.

Except as expressly provided in this agreement, the Practitioner makes no guarantees or warranties, express or implied. In no event will the Practitioner be liable to the Client for consequential or special damages. Notwithstanding any damages that the Client may incur, the Practitioner’s entire liability under this agreement, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the Practitioner under this agreement for all services rendered up until the termination date.This is the entire agreement of the parties, and reflects a complete understanding of the parties with respect to the subject matter. Client understands if any provision of this Agreement shall be held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. If the Court finds that any provision of this Agreement is invalid or unenforceable, but that by limiting such provision it would become valid and enforceable, then such provision shall be deemed to be written, construed, and enforced as so limited. This agreement supersedes all prior written and oral representations.
